WHAT WE ARE LOOKING FOR
Jefferson County is looking for a Children, Youth & Family Human Services Professional  to join the Child and Family Division with the Human Services Department whose mission is to enhance the quality of life for individuals and families living in Jefferson County by addressing their needs in a respectful manner and enable citizens receiving services to function as independently as possible, while acknowledging their cultural differences.

WHAT YOU WILL DO

The Children, Youth & Family Human Services Professional I/II position is focused on Family Teaming Worker duties.  The Family Teaming Worker will support the development and implementation of family team meetings using the Family Teaming model.  Work includes receiving and managing referrals; facilitating team meetings; completing assessments and plans of care.  This position requires a strength-based and family-centered approach.


SOME ESSENTIAL RESPONSIBILITIES            
You will be successful in this role by completing the following tasks and responsibilities:

  • Advocates for and supports permanency outcomes for children and youth in out-of-home care, ensuring they achieve stable and lasting placement.
  • Collaborates in staffing, family team meetings, and other multidisciplinary meetings to ensure informed decision making and to facilitate necessary services in the best interest of the children, youth and families.
  • Documents and reports interactions in compliance with departmental policies and Wisconsin State Statues accurately and timely including but not limited to assessments, plans, evaluations, reviews, progress notes, and reports.
  • Family Teaming Worker: Proficient knowledge and application of Family Find and Family Teaming models.  
Qualifications:

Requirements:

  • Level I: Bachelor’s degree in social work, Human Services, or related field; or an Associate’s degree with at least five years of professional experience in social work or related field; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position.
  • Level II: Bachelor’s degree in social work, Human Services, or a related field, plus to two three years of professional experience in social work, child welfare, or a related field; or an equivalent combination of education and experience sufficient to demonstrate advanced skills in case management and crisis intervention for complex cases.

Preferred Requirements:

  • Level I: CHIPS Case Manager, Youth Justice Manager, and Family Teaming Worker Certifications preferred.
    Level II: CHIPS Case Manager, Youth Justice Manager, and Family Teaming Preferred knowledge/training and advanced understanding in trauma-informed care                             

          Wage & Benefits                         
The starting hourly wage range is $29.93 - $31.65/hour depending on education and experience.
Potential to earn up to $38.49/hour dependent on successful annual reviews.
